Client Forms in North Kingston

For your convenience, our client forms are available online. These forms can be printed and filled out from home and returned via e-mail to reception@vvc.vet or via fax to (401) 223-6404 prior to your visit. You can also bring in the hard copy of your filled out form to your pet’s appointment.

The Village Veterinary Center is located on West Main Street in the historic downtown district of Charming Wickford Village.

New Client Form

Is your pet new to The Village Veterinary Center? We love meeting new patients and their owners and getting to know them on a personal level. All we need is for you to fill out the following New Client Form for your pet, and we’ll take care of the rest!

Surgical Consent Form

Before we can perform any surgical procedure on a patient, we must receive written permission/consent from the pet owner. If your pet has an upcoming surgery, and someone else will be dropping them off, please sign the following Surgical Consent Form and send it with that person.
